How do I become a Patient?
Appointments are by referral only made by either your family doctor/nurse practitioner, optometrist or other specialist
How long before I receive my appointment once the referral has been made?
Wait times may vary depending on the reason for the referral. By CPSO standards, a notice will be sent to your referring doctor with the triaged wait time. Please check with your referring doctor.
Will my appointment be covered by ohip?
Consults are covered by OHIP, however, if there are any additional costs, this will be discussed with the physician prior to proceeding.
What do I need to bring to my appointment?
You will need to bring the following items:
Valid OHIP card, list of any medications, glasses (if you wear them) list of any previous eye treatments or conditions
How long will my appointment take?
Your appointment may last 1– 2 hours
